Free Flu Vaccination now available.

In order to make this process as easy and efficient as possible, we have setup an online consent form which you can find at:

We would ask you to please circulate this link to parents/guardians by way of WhatsApp, Text, Email, Social Media and your website notice board. Please send the link by as many channels as you can, as this really helps to increase uptake rates. 

When sending the email to parents/guardians you might also please attach the letter and two information leaflets that I have attached to this email. 

A few parents in a couple schools have had issues accessing the links so to combat this here is a link directly to a typeform, same form but just not on our website. This should work okay for parents;

Another fix would be to manually search edupharma.ie and select "I am a parent" this will bring them to the form.
